Social Work will perform duties as assigned for
*Bereavement:* Provides emotional care to terminally ill patients and their families as well as grief support to the family after the death of the patient.
Ability to work with people of various religious faiths and cultures.
Responsible for developing bereavement programs and overseeing current programs, including social activities and grief support groups *Social Work:* Responsible for consultation and direction of Social Service in Hospice and Home Health.
Provides orientation and education to the spiritual counselors, volunteers and nursing staff.
Develops, organizes and implements programs, policies and procedures, conducts case work services and counseling and recommends functions necessary to ensure overall operational viability.
Participates in performance improvement and continuous quality improvement (CQI) activities.
_*Essential Functions & Responsibilities:*_ * Promotes the mission, vision and values of the BMHD.
* Core Hospice discipline.
To be knowledgeable about state and federal regulations related to social work * Develop a personalized plan of care that addresses the psychosocial needs of each patient and family.
* Be knowledgeable of Community Recourses and know how to make connections and referrals where needed.
* Ongoing orientation and education to the spiritual counselors and volunteers * Helps plan/organize Hospice fundraisers * Plan, implement and coordinate Grief Support Group * Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications, attending webinars, establishing professional networks, participates in professional organizations.
* Home Health psychosocial needs of each patient and family * Any additional duties pertaining to the daily operations of the Hospice Agency as assigned by the Hospice Director.
* Customer Focus _– _dedicated to meeting the expectations of the internal customer.
Demonstrate sensitivity, empathy, concern and respect for others.
Act with the patient care in mind, gain trust and respect by building and maintaining effective relationships.
* Maintain confidentiality _– _in accordance with HIPPA, even after a death of a Hospice patient * Demonstrate sensitivity, empathy, concern and respect for others.
Act with the patient care in mind, gain trust and respect by building and maintaining effective relationships.
* Maintain patient confidentiality at all times by adhering to the federal HIPAA regulations.
* Represents the organization in a positive and professional manner at all times.
* Maintains self-motivation and sustains a high level of momentum.
* Able to successfully prioritize when faced with requests, demands and deadlines.
* Knowledge of customer service concepts.
* Must be extremely detail oriented and able to perform social work duties as well as detailed record keeping.
* Proven ability to communicate effectively, both in written and oral form.
* Ability to communicate effectively in English, both verbally and in writing.
* Proven ability to work as part of a team, in a fast-paced production-oriented environment.
* Proven presentation skills.
* Expresses excellent judgment, discretion and decision making abilities.
* Maintains a sensitive, fair and impartial approach to dealing with employees.
* Supports and maintains a culture of safety, quality and positive patient experience.
* Working knowledge of relevant regulatory requirements, CMS, National Patient Safety Goals (NPSGs), National Hospice and Palliative Care (NHPCO), and continued readiness related to healthcare or hospital standards.
* Ability to prioritize, highly organized.
_*Education Requirements:*_ *Required:* * High School Diploma or GED Equivalent * Bachelor’s Degree in Social Work supervised by LCSW *Preferred:* * Great Social Skills, and learns new skills quickly.
* Working experience in hospice or medical, critical access hospital setting * Master’s Degree in Social Work _*Certification / License Requirements:*_ *Required:* * Basic Life Support (BLS) certified by American Heart Association (AHA).
* MUST obtain within 60days of hire.
